Participants in the program are responsible for managing their care providers and must report back to the Home and Community Care Support Services.

What are the responsibilities of the patient or SDM?

Under the Family-Managed Home Care (FMHC) program, patients or their Substitute Decision-Makers (SDMs)—such as a parent, spouse, or legal guardian take on significant responsibilities. These include overseeing both the care and financial aspects of home-based services.

They are responsible for:

  • Identifying the care needs of the patient and planning when and how services should be delivered

  • Recruiting, scheduling, and supervising care providers, including setting up contingency plans when needed

  • Acting as an employer, often for multiple providers, and managing contracts, schedules, and performance

  • Managing financial operations such as maintaining the SDC bank account, paying service providers, withholding taxes, obtaining insurance, and keeping accurate records

  • Submitting reports to Home and Community Care Support Services (ontario health at home), ensuring compliance with program guidelines
